Who do I request records from at Health New Zealand?

Requesting your records can bring up a range of emotions. Learn where you can find support to help you on your journey. 

You can make a records request to the Health NZ regional location that has your records

If you know which hospital or facility you were in, and it is still operating, it is quicker to make your request to the relevant Health NZ location(s). These used to be called District Health Boards or DHBs.

Or you can request records from Health NZ’s head office

You can request records from the Health New Zealand – Te Whatu Ora (Health NZ’s) head office if:

  • you are not sure which hospital or facility you were in
  • the hospital or facility you were in has closed (for example, Kingseat Hospital, Porirua Hospital)
  • you were in more than hospital or facility and you do not want to make record requests to several Health NZ locations.
